FR001 Twenty-two Questions
The aim of the following questionnaire is to help us build a clear picture of the kind of person you are and to assess the style of Magick that might be most appropriate. If you have any doubts about answering the questionnaire then please say so. You may prefer to meet someone from the Society before completing it - that is fine by us. We also recommended that you have a look at the book list on the associate membership card before completing the questionnaire, as you may feel that our style of Magick is not for you. If you do decide to return the questionnaire, please include a current photograph.
On a separate sheet of paper copy out the question and then in your clearest handwriting or typing answer as many of the questions as you wish. Do not rush, spend some time and care thinking about your reply. If you want to write it as an essay about yourself and what leads you to the study of magick, that is fine by us.

Costs
The Society is non-profit making and run by volunteers. Donations towards the running costs of the Society are always needed. In addition we would ask that when you return this questionnaire you include the £5 associate membership fee for the Society, if you have not already done so. This money will be used to pay postage on regular mailouts and newsletters.
We can also offer a short foundation course in the basic techniques of magick. This includes guidance towards the completion of a foundation magical year, with appropriate banishings etc. and introduction to the astral temple practice. The course includes some personal tuition, where appropriate with experienced magicians and also feedback on any written work from diaries etc. If you feel you are already adept at this kind of thing, then advice and guidance on more advanced magical directions can sometimes be forthcoming. There is no fixed cost for this although we hope that one day you will be willing to render a similar service to other beginners. Furthermore you should keep a record of the numbers of documents sent to you, including letters and aim to make a donation to the Society for at least the cost of photocopying and origination at approximately 20p per printed side.
